The United States Bankruptcy Court for the District of New Jersey has established June 23, 2009 as the last date for a creditor to file a proof of claim (except for a governmental unit which will have 180 days from the bankruptcy petition date which was February 23, 2009). Attached is a proof of claim form for your use. The completed and signed proof of claim form must be received by the Bankruptcy Court clerk's office by June 23, 2009. The mailing address is:
